

Enablement Project

Location Residents in Cornwall.
Aims This project will support adults with mental health issues who fall into the low to moderate fair access to care services (FACS) and/or who are not eligible to receive a service from the Community Mental Health Teams(CMHT) or who are not receiving a regular service from the CMHT.
The Enablement Project is a partnership between Pentreath, The Rural Community Link Project and West Cornwall Mind. It provides short-term (3months) support to help enable you to achieve your goals, to help people become involved in sociable Leisure or Recreational activities, Training or Volunteering. Examples of what clients are currently benefiting from are as follows:
• Support in attending recreational activities to build fitness and confidence • Gaining independence through increased social activities • Advice and guidance on beginning short courses • Encouragement and support to try new things • Support in beginning volunteer placements • Signposting to other providers
As a client, you will be allocated your own Community Enablement Worker who will meet with you and discuss your wishes before drawing up an action plan that you will both work through over a 12 week period. Together, you will work out what you would like to achieve in the short, mid and long term and how you will achieve it. The most important aspect being that whatever you choose is sustainable and realistic for you to continue with into your future.
