We have three workers covering all of Cornwall and we work with clients for 6 weeks in a person centred and practical way to support them to move forwards. This might involve: 1:1 sessions (around confidence building, building assertiveness skills or anxiety and stress management); supporting clients to access other relevant services and/or community groups and provisions; and small goal setting. The idea is that we help clients to access support, or deliver it ourselves where appropriate, that will create sustainable change. We work holistically with issues that the client may raise.
Clients who are 16 years and older are able to access the project, with no upper age limit to the provision.
We follow up our intervention with the client three months after the end of our provision to check that the support put in place has been useful, effective and the client continues to be moving in the direction they want to go in.
Referrals for this project will come from the Community Gateway Helpline Workers and Primary Care Networks.
