We provide support at your GP surgery to link you with activities and other support to improve your mental health and wellbeing. Please speak to you GP if you would like to access this project.
Social prescribing is a key component of Universal Personalised Care.
A Social Prescribing Link Worker (SPLW) gives people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support. SPLWs are often based in a GP surgery but can also be based within the community. They accept referrals from GPs and clinical staff within the surgery but also accept self-referrals and referrals from people in the community.
SPLWs also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.
Pentreath currently have Social Prescribing Link Workers based at the following locations:

Mullion and Constantine Group Practice
Bridge Medical Centre, Wadebridge
Wadebridge and Camel Estuary, Wadebridge
Bottreaux Surgery, Boscastle
We are also part of a partnership, funded by the Department of Health that provides Social Prescribing Link Workers in the locations below.
